It’s only being measured if the tracking pixel is being loaded, and for that, the email client needs to load images. So, you need to make sure that the receiver wants to see images in the e-mail.

For example, I have my mail client (gmail) configured to only load images for trusted sources. When I receive an email from an untrusted source, I don’t see images and I have to click a link to load images and/or to trust the sender. Now, I won’t click that link if the email is perfectly readable and I don’t seem to miss any information. Meaning, I opened the mail but it’s not reflected in the open rate.

So my tip is to add at least one real image with a place holder text that makes people curious to see what’s on the image.
